The following documents are required to renew the school-based program:

• Application for Participation: This document is only necessary for the first year of enrollment. It must be signed by the principal.
  

• Annual Memorandum of Understanding: This document must be signed by the principal and include the names of the lead teacher, as well as two other adults who are aware the program is taking place.
  

• Student Roster or a letter declining to release students' names: Learning for Life will accept either a student roster signed by the principal or a document on school letterhead signed by the principal that states:

"Because of privacy reasons, our school does not wish to release the names of students. There are [number] students participating in the program. Please accept this letter in lieu of a roster."
